Contestants' Paperwork

For 2015 Competition (to take place March 1, 2014)


  First, determine if you are a MISS or a TEEN:

MISS:   Must be 18 - 26 on February 1st, 2015, Birthday should fall between 2/2/1988 and 2/1/1997

TEEN:  Must be 14 - 17 on February 1st, 2015, Birthday should fall between 2/2/1997 and 2/1/2001

 

  The paperwork provided below is a .pdf file.  You can print it up, or retype it.   If you need a particular form in "word" format, contact Cammie (clsanders@bellsouth.net) and it can be emailed to you.  
 
 

  HOW AND WHEN TO TURN IN YOUR PAPERWORK AND REQUIREMENTS !!  

Deadline 2-5-14

Things to turn in (Miss & Teen):

(1) Headshot Photo, at least 5x7, preferably color, no crowns (may also be emailed or put on CD as .jpg), happy photo please, not seductive

(2) Copy of Driver's License (unless you are too young to have a driver's license yet), or Florida picture ID

(3) Copy of Birth Certificate

(4) Proof of Residency [In the form of: voter's registration card; income tax return(s); current credit card, magazine subscription, telephone, electric, medical bills, rent receipts or other current invoices verifying my place of residence; automobile registration, verification from college or business, etc.]

 

  Special Note/Opening Number Outfit:  The Opening Number is anticipated to have a "disco" theme.  Select a current-day disco-inspired cocktail dress for the opening number.  We are not looking for a disco costume, we want you to look pretty in a cocktail dress (which you will remain in for the on-stage question competition).   Please select something fun, sparkly, colorful, festive, but still current-day looking.  Reasonable high-heels please, no insane platform shoes!  We want you to look PRETTY!!

Paperwork - Little Sister

*Print up the application to be a Jax USA Little Sister by clicking the above referenced link.  Be sure to also submit with your application the little sister fee, a photo, and a short bio of the little sister.

We can take lots of little sisters!

  When it is time for the pageant and its rehearsals, contestants will not be allowed to have parents, friends, helpers, or coaches at the rehearsals or back stage during the event.   Contestants should learn to do their own hair and make-up for this competition, because if they become our titleholders, they will have to do so at the state competition.  

No cameras or videos will be allowed during the competition.  The pageant will make sure there is a photographer and/or videographer there to capture the event, and you will be allowed to order from them.

The Pageant Committee / NFSO is not required to provide special assistance or training for the contestants, but we do try to help the contestants and give them guidance at our upcoming meetings, rehearsals, etc. upon request.
