Photographs - two 5 x 7 (non-crown) headshot, interview suit preferred, color photos preferred.

Copy of Birth Certificate

Proof of Residence (2 forms) showing that you live, work, or go to school for at least 6 months within the designated radius of eligibility for the pageant

Music for your Talent - 2 cassette tapes or 2 CDs, must be 2 minutes or less - no exceptions.  The music on your tapes or CDs should be the only music on that tape or CD!
