Instructions for completing the Contestant's Fact Sheet **This is your opportunity to demonstrate to the judges why they should consider you for the job of Miss Jax or Miss FC. You may list as many or as few items under each category as you desire except where indicated otherwise. Your only restriction is that it must fit on one page and that you do not use a font style smaller than 10" and no larger than 12" point type. "Times New Roman" is requested. --------------------------------------------------------------------------------------------- Name: State your name as you would like it listed in the program book. Date of Birth: Month, day, and year, i.e. October 10th, 1980. Education: List your most recent school first, indicate your status (freshman, sophomore, etc.), list your major or minor if applicable. List college and high school. Platform Issue: Only type the title of your platform, i.e. Celebrating Today's Youth. (All other info on your platform should be in your essay). Scholastic Ambition: Type the degree/education sought/achieved. If you have graduate college, list your undergraduate degree and the degree that you are seeking in high education, if applicable. Career Ambition: List your career goal(s) - but do not list more than three. Talent: Just indicated the type of talent (pop vocal, classical piano, slow lyrical dance, upbeat country clogging, etc.) and the name/title of the musical selection that you will perform to. (Don't give a detailed description. Scholastic Honors: List the most recent honors that you have received. A semi-colon should separate each item. Leadership Roles: A semi-colon should separate each item. Accomplishments: A semi-colon should separate each item. Interesting Facts: A semi-colon should separate each item. Employment: A semi-colon should separate each item. What three words best describe you? Use only 3 words here, do not elaborate. Why I should be Miss Jacksonville or Miss First Coast: Explain to the judging panel why you feel you should be selected as the titleholder. This is your opportunity to let them know why you are the right person for the job and what you have to offer. --------------------------------------------------------------------------------------------- **This entire form must fit on ONE page, and the preferred font is Times New Roman style. You must leave a 1 inch margin on all four sides of the page. Use your best judgment on what is most important for the judges to know about you. Use your space wisely. Any variations from this form will not be accepted. Your materials will not be retyped and the judges will receive your original work. Forms must be typed; handwritten is not acceptable. **This form will be given to the judges along with your platform statement/essay. You will notice that there is a required signature line at the bottom of the platform statement. When these two pages are put together, they will create your entire application for the job of titleholder. Your signature will cover the materials listed on both pages and verifies that everything on your forms is true and accurate. If it is proven that any information on these forms is not true, correct and factual, you risk losing your local title. |
