You may print off these on-line forms or you may retype these forms (or cut and paste into a word processing program), as long as all of the information is the same and the form looks relatively the same.   The FACT SHEET and PLATFORM ESSAY must be on one piece of paper each (you can not go into two pages or more).

Other Items to Turn In

• Photographs - two 5 x 7 (non-crown) headshot, interview suit preferred, color photos preferred.

• Copy of Birth Certificate

• Proof of Residence (2 forms) showing that you live, work, or go to school for at least 6 months within the designated radius of eligibility for the pageant

• Music for your Talent - 2 CDs, must be 2 minutes or less - no exceptions. Your talent music should be the only music on the CD!   Please, no cassette tapes, the quality is not as good and they are harder to dub onto the Master CD.

• Any Sponsorship(s)

• Any Ads / Patron Sales
